Terms of Service

Terms of Service

These terms govern use of the Stonebridge Health Services website, patient access tools, RCS messaging, SMS text messaging, phone, email, and other digital communication channels.

Effective date: July 6, 2026

Use of our digital services

Stonebridge Health Services provides digital tools to help patients and visitors request appointments, ask billing questions, review general care guidance, communicate with care-access teams, and receive service-related updates. By using our website or digital communication channels, you agree to use them lawfully and only for their intended purposes.

You are responsible for the accuracy of information you submit. Do not use our digital services to impersonate another person, interfere with system operation, attempt unauthorized access, transmit harmful code, or send abusive, misleading, or unlawful content.

Not for emergencies

Stonebridge digital channels are not emergency services and should not be used for urgent or life-threatening medical needs. If you believe you are experiencing a medical emergency, call 911 or seek emergency care immediately. General website content and digital messages are not a substitute for professional medical advice, diagnosis, or treatment from a licensed clinician.

RCS, SMS, short code, and long code messaging terms

When you opt in to receive mobile messages from Stonebridge Health Services, you agree that we may send RCS, SMS, MMS, short code, long code, toll-free, or similar mobile messages to the mobile number you provide. Messages may include appointment reminders, scheduling updates, intake follow-up, billing support, service notifications, care-navigation information, and responses to your requests.

Message frequency varies. Message and data rates may apply. Consent to receive text messages is not a condition of receiving care or purchasing services. Carriers are not liable for delayed or undelivered messages. Delivery depends on your mobile carrier, device, network availability, and supported messaging features.

You may opt out at any time by replying STOP. After you send STOP, we may send a confirmation message and then stop sending non-essential mobile messages to that number unless you opt in again. For help, reply HELP or contact Stonebridge Health Services using the contact options on this website.

Privacy and communication records

Use of our digital channels is also governed by our Privacy Policy. We may keep records of messages, appointment requests, consent status, opt-out requests, delivery status, and related metadata to operate our services, comply with law, and protect our systems.

Please do not send highly sensitive information, full medical records, payment card numbers, or urgent clinical details through ordinary text messaging. If secure communication is required, Stonebridge may direct you to a more appropriate channel.

Accounts, forms, and patient access tools

Some features may require sign-in, identity confirmation, or additional verification. You agree to keep account credentials private and to notify Stonebridge if you believe your account or phone number has been used without authorization. We may suspend or restrict access to digital services if we detect abuse, security concerns, or misuse.

Content and availability

We work to keep website content and digital services accurate and available, but we do not guarantee that every service will be uninterrupted, error-free, or available on every device, carrier, browser, or messaging platform. We may update, suspend, or discontinue digital features at any time.

Third-party services

Stonebridge may use vendors and communication providers to host, transmit, route, secure, or support digital services. Your mobile carrier, device maker, browser provider, and messaging platform may also have separate terms and privacy practices. Stonebridge is not responsible for third-party services outside our control.

Limitation of liability

To the fullest extent permitted by law, Stonebridge Health Services is not responsible for indirect, incidental, special, consequential, or punitive damages arising from use of, inability to use, delays in, or reliance on our website or digital communication channels. Nothing in these terms limits rights or responsibilities that cannot be limited under applicable law.

Changes to these terms

We may update these Terms of Service from time to time. The updated version will be posted on this page with a new effective date. Your continued use of our website or digital services after an update means the revised terms apply going forward.
